1. Get a contact lens fitting and/or eye exam at Forrester Eye Associates or bring in your outside prescription.

2. Purchase required boxes, as indicated below, within 90 days of the eye exam.

Purchase must be made by June 30, 2023.

Save 10 % when you buy an annual supply of select contact lenses without insurance.

The UPC from one box of qualifying lenses
Your sales receipt showing the lens purchase and purchase date (You order verification email or the shipping invoice receipt)
Your eye exam/lens fitting receipt from your eye care provider

Get an annual contact lens exam at Forrester Eye Associates or bring in your outside prescription.
Purchase an eligible supply of CooperVision contact lenses within the promotional period.
Visit the manufacturer’s reward website at www.coopervisionpromotions.com and complete the online reward form within the specified time period. Follow the instructions to complete your submission.
